  1. How do I obtain a new badge?
  1. Ask Human Resources for a blank badge. Take the blank badge to Reprographics to have it imprinted with your picture and name. As a final step, have our Facilities Manager encode and activate the new badge.
  1. How do I submit a change of address? Do I have to contact each benefits vendor separately?
  1. No, you only need to give the address change to Human Resources. They will take care of notifying all plan administrators of your new address.
  1. How do I change my weekly 401(k) contribution?
  1. You can change it over the phone by calling Fidelity Customer Service at 1-800-354-7125, or you can visit Fidelity's web site at http://www.401k.com.
  1. How can I borrow money from my 401(k) account?
  1. You can discuss your needs with a Fidelity Customer Service Representative by calling 1-800-354-7125, or you can visit Fidelity's web site at http://www.401k.com and log in to your personal account. Click on the menu selection for information on withdrawals and loans.
  1. When may I drop a dependent from my health plan coverage?
  1. You may drop a dependent during our annual open enrollment period held every year in the November timeframe. Changes become effective on January 1st of the following year. You may also drop a dependent if you have a family status change, such as a divorce, anytime during the plan year. Check with Human Resources for details.
  1. If I terminate my employment with the company, do I lose the vacation time I haven't used?
  1. No, you do not forfeit unused accrued vacation when you leave. Any unused, accrued vacation hours will be paid to you in a separate check approximately one week after your last day worked.
