- How do I obtain a new badge?
- Ask Human Resources for a blank badge. Take the blank badge to Reprographics to have it imprinted with your picture and name. As a final step, have our Facilities Manager encode and activate the new badge.
- How do I submit a change of address? Do I have to contact each benefits vendor separately?
- No, you only need to give the address change to Human Resources. They will take care of notifying all plan administrators of your new address.
- How do I change my weekly 401(k) contribution?
- You can change it over the phone by calling Fidelity Customer Service at 1-800-354-7125, or you can visit Fidelity's web site at http://www.401k.com.
- How can I borrow money from my 401(k) account?
- You can discuss your needs with a Fidelity Customer Service Representative by calling 1-800-354-7125, or you can visit Fidelity's web site at http://www.401k.com and log in to your personal account. Click on the menu selection for information on withdrawals and loans.
- When may I drop a dependent from my health plan coverage?
- You may drop a dependent during our annual open enrollment period held every year in the November timeframe. Changes become effective on January 1st of the following year. You may also drop a dependent if you have a family status change, such as a divorce, anytime during the plan year. Check with Human Resources for details.
- If I terminate my employment with the company, do I lose the vacation time I haven't used?
- No, you do not forfeit unused accrued vacation when you leave. Any unused, accrued vacation hours will be paid to you in a separate check approximately one week after your last day worked.
